Hall of Fame Nomination Form

Instructions for Nominators to complete the Nomination Package:

The deadline for submission of nominations is June 1st for consideration for the inductions in November of the same year.
As a Player:
born in Ontario or resided in Ontario during the period of outstanding performance over a lengthy period of time; The accomplishment(s) of the player must have included a playing career in a Baseball Ontario program.
As a Builder:
the individual must have been born in Ontario or have resided in Ontario during the period of outstanding contribution and must have demonstrated outstanding service to baseball over a lengthy period of time.
Teams will be selected that have represented with distinction either a particular community or the whole province and recognition of such team must exceed more than one year.
Nominations for “Special” category can include teams of a single year’s accomplishments and groups of players or builders who have made a significant contribution.
The Nominator, Supporters and/or Inductee may be requested to provide additional information for the citation. The Baseball Ontario Hall of Fame Committee shall make the final decision on the content of the citation to be used at the Induction Ceremony.
