Baseball Ontario Game Incident Report
This is the official incident report for Baseball Ontario tournaments, Eliminations, and playdowns.
It is to be completed by the umpire following the game where the incident(s) took place. Multiple incidents from the same game can be reported in one submission.
This form may also be optionally submitted for incidents in local games/tournaments, but be sure to also let your UIC/assigner know immediately via email or text.
